Boston Red Sox manager Alex Cora said on Sunday afternoon that starting pitcher Patrick Sandoval is dealing with left bicep tightness that he experienced following his latest rehab start with Triple-A Worcester.

"Patrick Sandoval is here today after experiencing tightness in his bicep. Obviously not what he or the club wants given his extended absence," Mac Cerullo of The Boston Herald posted on Bluesky.

"Patrick Sandoval was supposed to start for Worcester today but is experiencing discomfort in the bicep area. He came back to Boston to get it checked out. Payton Tolle was originally supposed to pitch Saturday or today, but he will be held out in case Sox have a rainout today," MLB.com's Ian Browne wrote on X.

Sunday's news is the latest in a string of bad injury updates for Sandoval.

The 29-year-old left-hander went 2-8 with a 5.08 ERA, 1.50 WHIP and 81 strikeouts across 79 2/3 innings pitched covering 16 starts during the 2024 season with the Los Angeles Angels and signed with the Red Sox that December.

Sandoval missed all of the 2025 season due to Tommy John surgery though and has yet to pitch with Boston.

"He felt some tightness in his day three, so he’s here just to make sure everything is ok," Cora said, according to a report by Cerullo. "I don’t know what’s next with him but obviously they want to check on him."

The southpaw allowed no runs in 3 1/3 innings pitched during his first rehab start on April 8 but surrendered five runs in two frames during his second outing with Worcester on Tuesday.
